- Health-related information — details you voluntarily share during a health screening or Good Faith Exam that are relevant to your treatment.
- Usage data — general analytics about how visitors use our site (pages viewed, browser type), which helps us improve the Service.

We use the information above to schedule and confirm appointments, communicate with you about your booking, support our clinical team in delivering safe care, and improve our website and services. We do not sell your personal information.

SurgIV is not itself a hospital or covered entity, but our affiliated clinicians handle health information you provide with the same care and confidentiality expected in a clinical setting. Health information you share during a screening is used solely to determine whether a treatment is safe and appropriate for you.
